Laptop Checkout Agreement made between ITO Media Services (Loaner) and Customer (student, staff, faculty)

LAPTOP CHECKOUT AGREEMENT


  1. Laptops are for use during Media Services hours.  NO OVERNIGHT checkouts are allowed.  You will lose checkout privileges if the laptop is held overnight. 
  2. LAPTOPS ARE NOT TO BE TAKEN OFF CAMPUS. 
  3. Laptops are available to checkout for four hours and can be renewed. To renew, bring the laptop back to the counter for scanning. 
  4. Student shall keep all loaned equipment described in good condition and working order and agrees to return the loaned equipment in its present condition (reasonable wear and tear excepted).  
  5. Student will promptly return all loaned equipment including AC Adapter to ITO Media Services no later than specified return date and time.   Late fees of $1.00 per hour will be charged until laptop is returned.  
  6. Student is responsible for any loss or damages to laptop and may be subject to actual replacement costs of the laptop and a hold on your student account. 
  7. If laptop is stolen, student will report the theft to the police at the time the theft occurs and inform ITO Media Services.  A copy of the police report documenting the theft of the laptop will need to be given to ITO Media Services.  Student will be billed for full replacement costs of the laptop if a police report is not done. 
  8. ITO Media Services reserves the right to place a hold on student’s account and ability to register for future classes or obtaining transcripts if student fails to return laptop or pay fines and replacement costs.  
  9. ITO Media Services will not be responsible for any student data.  It is the responsibility of the student to back up their data before loaned equipment is returned. When the laptop shuts down all information is lost. 
  10. ITO Media Services will keep student’s personal information private and may keep it on file for future checkouts during the academic school year.  At the end of the school year Media Services will shred the agreement form and student will need to fill out a new agreement form for the new academic year. 
  11. ITO Media Services reserves the right to the terminate loan agreement and retrieve loaned equipment to satisfy a higher priority need.  
  12. Both ITO Media Services and the student will inspect loaned equipment at time of checkout. Before the customer acknowledges receipt both parties will agree that all loaned equipment is in good working order.
